This is a registration form for your child for the party they are attending at Forest Freedom.

When you enter Wandle Recreation centre please turn left, walk alongside the pitches to the end and then turn right where you will see the entrance to our forest.

We can't wait to welcome you. Please be on time and in suitable clothing!
